You pay us. No community pays us. Ever.

Most senior placement services cost families nothing — because the community pays them when you move in, often a fee equal to a full month’s rent. Those services can only show you communities that have signed paying agreements with them.

Four out of five licensed assisted living programs in Maryland are small residential homes of fifteen beds or fewer. Most cannot afford referral fees. So they don’t sign. So families never hear about them.

Some of the best care in this state is in houses you will not find on a referral website.

We accept no money from any community, ever. So we can recommend a small residential home in Catonsville run by an owner-operator with two decades of senior care experience, exactly as readily as we can recommend a hundred-unit building. What we recommend is what fits.

Before we make any recommendation, we do the work.

  1. A Registered Nurse assesses your loved one — clinically, honestly, and in person. We tell you what care level they actually need today, not what a marketing brochure will imply they might get.

  2. We lay out the real options. Remaining at home with more support. A small residential home. A larger community. Memory care. Skilled nursing. Each one described honestly, including the ones we might not recommend.

  3. We model what each option costs over the next twenty-four months — including any long-term care insurance benefit your family holds, if applicable. Not a monthly rate. A two-year picture.

  4. We identify specific places worth your time. Not a list of twenty. A shortlist of three to five, each with a written reason we chose it.

  5. We verify every one holds a current Maryland license, and we hand you that verification in writing.

  6. We tour with you — scheduled at mealtimes or during activities, so you see the place as it actually is, not how it looks in a marketing photograph.

  7. We read the residency agreement before you sign it, and tell you what to ask.

  8. We are there on move-in day, and we check in at thirty, sixty, and ninety days.

Consultation

A nurse assessment, a clear determination of the level of care needed, and a written briefing on your real options with costs modeled over two years. For families who need to know where they stand before deciding anything.

Guided Placement

Everything in Consultation, plus a curated shortlist with licensure verification for each, accompanied touring, residency agreement review, presence on move-in day, and follow-up at thirty, sixty, and ninety days.

Full Transition Management

Everything in Guided Placement, plus the move itself: downsizing, a floor plan for the new space, estate and disposition coordination, home sale support, document assembly, family mediation where it is needed, and a complete clinical handoff to the receiving community.

Sometimes the right recommendation costs us a client.

EagleWings is a home care agency. Sometimes the right recommendation is that home care is no longer enough — which means telling a family to stop paying us for care.

We would rather tell you that than keep you as a client.

Before we begin, we give every family a written disclosure explaining exactly how we are paid, what we refuse, and how we protect you from our own interest. We would rather you read it than trust us on faith.

If we already know your loved one, we begin from a real foundation.

If your loved one is already receiving EagleWings home care, we already know them clinically. That means our Transitions engagement begins from a real foundation — not a first-time assessment. In practice, this often means faster clarity, better recommendations, and a smoother handoff to whichever community your family chooses. If you’re an existing client, mention that when you reach out.

The bright lines that protect you.

  • We won't take money from any assisted living community, memory care community, independent living community, nursing home, or continuing care retirement community. Ever.
  • We won't accept a “thank you” gift, a holiday hamper, or lunch at a community's expense. We won't attend “provider partnership dinners.” We won't put any community's logo on our materials.
  • We won't recommend any community that we haven't personally verified as licensed with the Maryland Office of Health Care Quality.
  • We won't recommend any community we haven't seen in person.

Common questions about Transitions.

Do you accept payments from communities?

No. Never. Every family receives written confirmation of this before engagement begins.

Do I need to be an EagleWings home care client to work with you?

No. Transitions is available to any Maryland family. Existing home care clients often engage Transitions when a loved one's needs progress beyond home care, but the service is not membership-gated.

What if I ultimately decide to stay home with more care?

That's a legitimate outcome. Our fee doesn't change based on what your family chooses. If Consultation surfaces that staying home with expanded support is the right answer, that's the answer.

How is this different from A Place for Mom or Caring.com?

Those services are free to families because the communities pay them. Their available communities are limited to those with paying agreements. Ours is paid by families and open to every licensed community in Maryland.

Where do you work?

Across Maryland. Most of our engagements are within Baltimore City, Baltimore County, and the surrounding counties, but we accept engagements statewide.

How quickly can we begin?

Most Consultation engagements begin within seven days of initial inquiry. Urgent situations — hospital discharge, crisis moments — we work to accommodate faster.
